SIENA Suppliers questionnaire

This questionnaire consists of 3 parts: In part I we ask you general information about your organization. In part II we ask you information about neural network products which are supplied by your organization. In part III, we ask you to name some end-users of neural networks. We want to use this information for a neural networks end-user survey.  We would greatly appreciate if you would take a few minutes to fill in this questionair. To do so, please download this page, fill it in, and send it by regular mail to SNN (add address).  If you need more (electronic) copies of part II and part III, please make them yourself.
