SIENA End-users questionnaire

This questionnaire consists of two parts. In part I we ask you some basic information. In part II we ask you information about the neural network applications in your organization. We would like you to provide us with this information about all (different) applications in your organization (if possible). Please make your own (electronic) copies of part II of this questionnaire. We would greatly appreciate if you would take a few minutes to fill in this questionair. To do so, please
download this page, fill it in, and send it by regular mail to SNN (add address).  If you need more (electronic) copies
of part II and part III, please make them yourself.
